Getting started
- Install the App and open it from your Shopify admin.
- Create a form, add fields, and set it live.
- Add the storefront block: Online Store → Themes → Customize → Product page → Add block → Apps → “Customization form”, then Save. (The App links you straight to the right place, and includes a step-by-step “Need more help?” guide.)
- Place a test order to see the answers appear on the order, confirmation email, and packing slip.
Common questions
The form isn’t showing on my product page
Themes don’t add apps automatically. Add the “Customization form” block to your product template and Save, as described above. Open the App and follow “Need more help?” for annotated screenshots.
How are add-on charges applied?
When a customer selects a priced option, a companion “Customization fee” line is added to the cart so the charge rides the order natively — this works on every Shopify plan (no Shopify Plus required).
Where do customer answers and uploaded files go?
Answers attach to the order as line item properties and appear across cart, checkout, email, order admin, and packing slip. Uploaded files are stored securely and linked from the submission. See our Privacy Policy.
